We are happy to process your AdultInfoCard order by mail. Just download and print out one mail-in order form for each person and fill in all of the requested information. Be sure to attach a photo of each person to the appropriate order form. If you need additional room for the Vital Information field (medications, allergies, additional contacts, medical conditions, etc.), just turn over the form and put it on the back or attach an additional sheet of paper with your application.  The calculations at the bottom of the form has a field for Sales Tax, but that ONLY applies to California residents (7.75%). The shipping choices and prices are shown below, so you just make your selection and include the shipping in your total cost on the form.

The order form is missing one important item — your choice of lanyard color!  You can choose from yellow, orange, red, green, purple, blue, navy blue, white or black.  Just write your lanyard color choice on the application.  If you do not make a lanyard color choice, the default color shipped will be yellow.
